In health care facilities, such as hospitals, nursing homes, or assisted treatment rooms, incident reports or accident reports are the completed forms to record details of unusual events occurring in facilities, such as injury to the patient. The purpose of the incident report is to document the exact details of the event as they refresh in the minds of those who watched the event. This information may be useful in the future when dealing with issues of liability arising from the incident.

Generally, according to health care guidelines, reports should be filled in as soon as possible after the incident (but once the situation has stabilized). In this way, the details written in the report are as accurate as possible.

Most of the reported incidents are written involving accidents with patients, such as falling patients. But most facilities will also document incidents where a staff member or visitor is injured.

In the event of an incident involving the patient, the patient will often be monitored for a period of time after the event (as may occur again), which may include taking vital signs regularly.

The latest guidebook for the International Classification of Disease Index, 10th Edition, Clinical Modification (ICD-10-CM) has been, in Chapter 20 (External Causes of Morbidity, Code V00-Y99), part of the external cause code to identify and track the occurrence of medical errors and certain serious surgeries and other serious events, which can be malpractice. The federal and state governments use never-inclusive lists as the basis for quality indicators and country-based reporting systems. Some of them are: Y65.51 (Performance of wrong procedure or operation in right patient); Y65.52 (Performance of procedures or surgery in patients not scheduled for procedures or surgery); and Y65.53 (Performance of the correct procedure or operation on the wrong side or wrong body part of the patient).
